Adjusted Body Weight Calculator
Calculate Devine ideal body weight (IBW), adjusted body weight (AdjBW), and percent of IBW for dosing-oriented scenarios.
Substantially above IBW reference. Dose strategy should follow medication-specific guidance.
At >=120% IBW, this tool surfaces AdjBW as the common default for dosing calculations.
Actual Weight Relative to IBW
AdjBW is often considered when actual body weight is at or above 120% of ideal body weight.
| RANGE | CATEGORY |
|---|---|
| 0-100 | Below IBW Actual body weight is below ideal body weight reference. |
| 100-120 | Near IBW Actual body weight is within roughly 100-120% of IBW. |
| 120-140 | AdjBW Commonly Used In many dosing protocols, adjusted body weight is considered from >=120% of IBW. |
| 140-160 | High Above IBW Substantially above IBW reference. Dose strategy should follow medication-specific guidance. |
| 160+ | Very High Above IBW Large divergence from IBW reference. Use clinician or protocol-driven dosing pathways. |
Formula Used
This calculator uses:
IBW (Devine) = 50 + 2.3 x (height in - 60) for men
IBW (Devine) = 45.5 + 2.3 x (height in - 60) for women
AdjBW = IBW + 0.4 x (Actual BW - IBW)
Many hospital dosing workflows apply AdjBW when actual body weight is at or above 120% of IBW, though medication-specific protocols can differ.
Use With Caution
This calculator is educational and does not replace medication-specific clinical guidance. Drug properties, renal function, age, and diagnosis can change the appropriate dosing weight method.
For non-dosing weight planning, use the Ideal Weight Calculator and Weight Loss Calculator.
To set intake from maintenance energy, pair with the Calorie Calculator and Calorie Deficit Calculator.
